ATTENTION APPOINTMENT SCHEDULERS!!! Before or after you schedule your appointment, please click on the patient forms link on our home page, and print out BOTH forms, fill them out entirely, and bring them with you to your appointment.

If you schedule an appointment, please make sure that if you need to cancel or re-schedule your appointment you give us AT LEAST 24 hour notice. If you fail to give us 24 hour notice, there will be a $25 cancellation fee added to your next visit. NO EXCEPTIONS!!!

When scheduling an appointment, we only accept 2 family members per day. Please keep this in mind!! If you attempt to schedule more than 2 family members on the same day, you will receive an email or phone call letting you know that you will need to re-schedule 1 or more of your appointments.
